Applicant's Date and Place of Birth: Date of birth must
be written in this format:
Day/Month/year. Spell out the month -- Example: 15 June 1970 Place of birth: City/Town, District/Province, Country -- Example: Munich, Bavaria, Germany. |

| 3- Applicant's native country, if different from country of birth: If the applicant is claiming Nativity in a country other than his or her place of birth, this country (instead of the country of birth) must be listed. Nativity in most cases is determined by the applicant's place of birth. However, if a person was born in an ineligible country but his/her spouse was born in an eligible country, such person can claim the spouse's country of birth rather than his/her own. Also, if a person was born in an ineligible country, but neither of his/her parents was born there or resided there at the time of the birth, such person may be able to claim nativity in one of the parents' country of birth. | ||

Name, Date and Place of Birth of Applicant's Spouse and Children (if any):
On your application you must list your spouse, that is husband or wife,
and all unmarried children under 21 years of age. You must list your spouse
even if you are currently separated from him/her. You must list ALL
your children who are unmarried and under the age of 21 years.
Your spouse may still submit a separate application, even though he or she is listed on your application, as long as both applications include the required photographs (see below) and details on all family members. Submitting separate applications for you and your spouse double your chances of winning. |

Applicant's Mailing Address: on the application we use our mailing
address. Using our address is very helpful for people who 1) want
to protect their privacy 2) may change their mailing address in the future
3) the mailing system in their country is not reliable or slow.
You must provide us with your complete mailing address and contact information. Since it will be to that address the notification letter will be sent if you win. Our office will contact all winners to verify their address, and send their notification letters by express or priority mail. |

Photograph ( Read Very Carefully ):
Include ONE recent "passport style" photograph of you, one for your spouse
if any, and one for each child (unmarried and under age 21) if any. Each family
member must have a separate photograph. Group or family photos will not
be accepted; there must be a separate photo for each family member. The
name of the applicant or a family member must be written
on the back of his or her photo.
You can send to us your photo as a digital image via email as an attachment, or through the online application. In addition to the above requirements, digital images must conform to the following specifications: For Digital Photos (photos taken by a digital Camera): --The image must be in the Joint Photograpphic Experts Group (JPEG) format. --The image must be either in color or graayscale; monochrome images (2-bit color depth) will not be accepted. --If a new digital photograph is taken, itt must have a resolution of 320 pixels wide by 240 pixels high, and a color depth of either 24-bit color, 8-bit color, or 8-bit grayscale. --The maximum image size accepted will be sixty-two thousand five hundred (62,500) bytes. For scanned Photos: --The image must be in the Joint Photograpphic Experts Group (JPEG) format. -- The print must be 2 inches by 2 inches (50mm x 50mm) square. --It must be scanned at a resolution of 150 dots per inch (dpi); the image must have a resolution of 300 by 300 pixels, with a color depth of 24-bit color, or 8-bit color, or 8-bit grayscale. --The maximum image size accepted will be sixty-two thousand five hundred (62,500) bytes. |
